Metformin HCl
Indications/Uses
Adjunct to diet & exercise to improve glycemic control in patients w/ type 2 DM. May be used concomitantly w/ sulfonylureas or insulin to improve glycemic control.
Dosage/Direction for Use
Monotherapy or in combination w/ other oral antidiabetic agents Initially 500 mg once daily, may be increased in increments of 500 mg every 10-15 days based on blood glucose measurements. Max: 2,000 mg daily. If glycemic control is still not achieved, max dose: 3,000 mg daily. Combination w/ insulin Initially 500 mg once daily.
Administration
Should be taken with food: Take w/ evening meal. Swallow whole, do not chew/crush.
Contraindications
Hypersensitivity. Diabetic ketoacidosis & pre-coma; renal failure/dysfunction (CrCl <60 mL/min); acute conditions w/ potential to alter renal function eg, dehydration, severe infection, shock; acute or chronic disease which may cause tissue hypoxia eg, cardiac or resp failure, recent MI, shock; hepatic insufficiency, acute alcohol intoxication, alcoholism. Childn.
Special Precautions
Lactic acidosis. Monitor CrCl &/or serum creatinine levels prior to initiating treatment & regularly thereafter. Discontinue use prior to, or at the time of iodinated contrast administration & not reinstituted until 48 hr afterwards & only after re-evaluated to normal renal function. Withhold therapy at least 48 hr prior to elective surgery w/ general spinal or peridural anaesth. Galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. May affect on the ability to drive & use machines in combination w/ other antidiabetics (sulphonlyureas, insulin or meglitinides). Pregnancy & lactation. Elderly.
Adverse Reactions
GI disorders eg, nausea, vomiting, diarrhoea, abdominal pain & loss of appetite. Taste disturbance.
Drug Interactions
Increased risk of lactic acidosis w/ acute alcohol intoxication & diuretics, especially loop diuretics. May lead to renal failure w/ intravascular administration of iodinated contrast agents. Drugs w/ intrinsic hyperglycemic activity [eg, glucocorticoids (systemic & local routes) & sympathomimetics].
